It is a new process that facilitates and improves the experience of travelers in their transit through the immigration, health and customs services of the Republic of Cuba.

The form can be accessed through the following link: https://www.dviajeros.mitrans.gob.cu

This form must be filled out by all travelers, whether Cuban or foreign, who enter to the Republic of Cuba through Airports, Ports and Marines.

This single form replaces the three forms currently required: International Boarding and Disembarkation Card, Customs Declaration Form, and Traveler's Health Affidavit. Now all this information is in one single form.

Details

As of January 1, 2022, the physical forms can no longer be used; Digital filling will be mandatory from this date. From November 15, 2021 to December 31, 2021, a test of the technology to be used will be carried out in which the use of the physical form will be exceptionally allowed.

The QR code will be required for passengers to enter the Republic of Cuba when passing through the Sanitary and Customs authorities; this code is the one that validates that the form has been filled out correctly.
