As of January 1, 2022, the physical forms can no longer be used; Digital filling will be mandatory from this date. From November 15, 2021 to December 31, 2021, a test of the technology to be used will be carried out in which the use of the physical form will be exceptionally allowed.
The QR code will be required for passengers to enter the Republic of Cuba when passing through the Sanitary and Customs authorities; this code is the one that validates that the form has been filled out correctly.
This single form replaces the three forms currently required: International Boarding and Disembarkation Card, Customs Declaration Form, and Traveler's Health Affidavit. Now all this information is in one single form.

What requirements are necessary to complete the form?

Enter the official page of the digital form D'VIAJEROS, enabled in Spanish and English; from any device: mobile, tablet or computer.

At the end of filling in the data requested and clicking on the "NEXT" button, a last SWORN DECLARATION screen will open, in which you can read the following information: "I declare that all the information provided in this form is true and complete, I understand that any omission or imprecision violates the provisions of the authorities of the Republic of Cuba."
Check the "I agree" box. It is important to check this box otherwise the "NEXT" button will not be activated.

Once the D'VIAJEROS form is completed, a PDF file with the QR code will be sent to the email indicated in the form, which must be shown (either on a device or printed) to the personnel accredited in Cuba, upon entering the country.

Opening hours: The digital form D'VIAJEROS, to complete the Affidavit, can be accessed 24 hours a day, 7 days a week, 365 days a year.
Resolution time: Immediately, when completing the online form, you receive an email with a PDF file containing a QR code, which you must show upon entering Cuba, either directly on your mobile device or printed.
